Information I need to file an Ohio Unemployment Compensation claims

If you are unemployed and wish to file for unemployment compensation benefits in Ohio, it would be advisable to know as to what all information and documentary proof you would be required to produce while filing an application.
The application is filed with and the benefits are given out by the Ohio Department of Job and Family Services.
The documents and information that you would be asked to provide would include:
1. Your Social Security Number and either your driver’s license or state ID number.
2. Your full name, address, telephone number and email address.
3. The name, address, telephone number and dates of employment with each employer whom you worked for during the past six weeks before your unemployment.
4. If you had worked outside the state of Ohio within the previous 18 months then the name, address, telephone number and dates of employment with each such employer whom you worked for outside the state for the past 18 months have to be provided.
5. If you have done military service in the past and were associated with any branch of the United States armed forces within the past 18 months then you need to provide your discharge papers.
6. If you were a federal government employee in the past 18 months then the SF-8 form or SF-50 form that you were supplied with by your government employer at the time of your discharge needs to be provided.
7. For each employer that you worked with in the past, you have to provide the reasons why you were laid off or fired or quit from that employment. There is also a requirement to provide details regarding your regular occupation, work experience and any job skills that you may have.
8. If you are claiming benefits for any dependents then the names of any dependents and their social security numbers and dates of birth have to be provided along with your application. If you are married then you have to provide your spouse’s name and social security number even if you are not claiming benefits for them.
9. If you are not a United States citizen or then your alien registration number and the expiry date for your work authorization has to be provided.

10. If you are claiming benefits for a dependent child, then the dependent needs to be under 18 years of age. If the child is over 18 years old then it has to be proved that he or she is unable to work because of a permanent mental or physical disability. It would be a good idea to prepare and keep the documentary proof that supports this claim. The other requirement is that for the 90 day period preceding your benefit year beginning date, it should be proved that you have paid more than half of the cost of supporting that dependent. The dependents will not be allowed benefits if your spouse has already filed for unemployment benefits which included claims for such dependents as well.
11. For claiming benefits for a dependent spouse it has to be proved that you are legally married and the spouse is living with you for the 90 days preceding your benefit year beginning date. They also need to have had income of less than 25 percent of your base period average weekly wage and more than 50 percent of the money required to support them must have been provided by you.
